Athletic Participation Packet
All students trying out for a school sport are required to turn in an athletic participation packet prior to the start of each school year.

Student-athletes will not be allowed to participate in any practices, scrimmages, or games until their athletic participation packet is completed and on file with the athletic department. Forms may be downloaded below.
The athletic participation packet consists of the following forms:
- Sports Pre-participation Physical
- Code of Ethics
- Athletic Contract Page of the Handbook
- Athletic Transportation Form
- COVID-19 Waiver
Student Athlete Grade Check
BCLHS student athletes are expected to maintain a minimum 2.0 grade point average while playing on any team. Students who do not meet standards may be placed on probation. It is the student's responsibility to submit a completed Grade Check Form to their coach every two weeks during the regular season. The form may be downloaded below.
Athletic Fees
All student-athletes are required to pay a $150 sports fee for each of the sports in which they participate. This fee is charged to the student's FACTS Tuition Management account at the beginning of the sports season. In addition, individual sports require fundraising to offset costs of spirit packs, tournaments, etc.
